Carestream Health Inc., (“Carestream”) expects that its Suppliers will conduct their business in compliance with their local environmental, health, and safety regulations. Products supplied to Carestream are to be in compliance with national and international regulations. Carestream must be able to demonstrate environmental, health, and safety compliance of end-use products to our customers and to regulatory authorities. To do this, we must obtain data and declarations from our suppliers and expect that our suppliers are actively working with their suppliers to ensure compliance traceability throughout the supply chain. Accurate data is necessary in order to pay appropriate material collection and disposal fees. Carestream uses internationally recognized material declaration questionnaires and other survey methods in order to obtain this data.
The European Union EU RoHS Directive, REACH Regulations and Waste Framework Directive (WFD), and other country regulations require Carestream to obtain detailed compliance evidence for restricted materials from suppliers for Medical Equipment. To meet these regulatory requirements, Carestream requires our Suppliers respond promptly to declaration requests sent from Carestream by email via Benchmark ESG™ Gensuite®. Benchmark ESG™ Gensuite® is a comprehensive cloud-based software suite that enables companies to implement robust cross-functional Environmental, Social, and Governance (ESG) Solutions. Benchmark ESG™ Gensuite® includes a free and easy regulatory compliance software tool to manage declarations for RoHS, REACH, and other restricted substance legislation. Instructions for Gensuite will be included within the survey.

For Suppliers of (US) Services
Suppliers that provide on-site services including but not limited to machine installation, plumbing repair, HVAC maintenance, electrical work, testing, construction/demolition, surveying, etc. are required to have the following minimum types and amounts of insurance coverage:
Required limits may be secured via a combination of primary and umbrella/excess policies. All coverages must include a waiver of subrogation in Carestream’s favor from the respective insurers. A certificate of insurance evidencing the required insurance must be provided prior to entering the Carestream facility (a standard Acord Form is acceptable). Additional required coverage will be dependent on services provided (e.g. Professional Liability, Errors & Omissions, etc.).
